What is a Community Health Worker (CHW)?

Community Health Workers are dynamic, frontline public health workers who have a very close understanding of and relationship with the community that they serve. Often, they grew up and still live in the community that they now work in, which helps them build trust amongst their patients and those they serve.

Community Health Workers act as links from healthcare systems to communities, helping to connect people to healthcare, social services, and everything they need to live a healthy life. CHWs improve the way our healthcare systems can deliver care by making them more culturally competent and helping to improve quality.

WIN Network: Detroit’s Community Health Workers help connect moms and families to resources such as food, housing, and transportation. They work with women one-on-one to make sure moms are ready for a healthy pregnancy, birth, and baby. Our CHWs also work with moms to create vision boards that lay out life plans and goals, and strategies to reach these goals.

What does a WIN Network CHW do?

Every WIN Network mom has a Community Health Worker (CHW) for support until their baby’s first birthday. WIN Network: Detroit’s Community Health Workers (CHWs) co-facilitate group prenatal care sessions alongside Certified Nurse Midwives – providing education and helping moms and families prepare for the changes of pregnancy and postpartum. Outside of group sessions, CHWs work one-on-one with moms to connect them and their family to essential resources such as food, housing, and transportation. They work with you to support healthy pregnancies, births, and babies and make sure you get the care you need.

What does a WIN Network Midwife (CNM) do?

Henry Ford Certified-Nurse Midwives (CNMs) don’t just deliver babies. From puberty to menopause, they offer a full range of services known as “well-woman gynecology,” along with a commitment to provide care that uniquely meets the individual needs of each patient. Our Certified Nurse Midwives practice at Women’s Health clinic locations and hospitals throughout Henry Ford locations in southeast Michigan. Certified Nurse Midwives provide care for:

• Prenatal care
• Postpartum care
• Menopausal care
• Family planning and birth control
• Breastfeeding education and support
• Preconception counseling and exams
• Teen education about body processes
• Treatment of minor gynecological infections
• Testing for infections, including those which are sexually transmitted
• Annual gynecologic exams (including teens), pap smears and breast exams
• Special needs, such as a baby with Down Syndrome, cleft lip and/or palate
• Birth control education and prescriptions, including: diaphragms, injections, IUDs, natural family planning and pills

Where is WIN Network: Detroit’s group prenatal care offered?

Our group prenatal care is currently held at Henry Ford Medical Center – New Center One for mothers delivering at Henry Ford Hospital. In addition to our long standing site in Detroit, groups are now offered at new community based locations, including the Center of Family Health in Jackson and the CHASS Center in Southwest Detroit—both Federally Qualified Health Centers (FQHCs) that expand access to high quality prenatal care for communities with the greatest need.

Who can join WIN Network: Detroit’s group prenatal care?

Any birthing person who meets the eligibility criteria (receiving care from a Certified Nurse Midwife and delivering at Henry Ford Hospital) can join! WIN Network: Detroit targets African American women, aged 18-45 for group prenatal care and extra support from a Community Health Worker.
