Henry Ford Health is committed to maintaining accurate, secure, and confidential patient information. As we continue integrating former Ascension hospitals and care sites, we offer several ways to access your medical records.

Urgent requests for medical records

If you need medical records urgently, contact the Health Information Management Department (Monday–Friday, 8:00 a.m. – 5:00 p.m.):

How to access your medical records through MyChart

Henry Ford Health allows you to securely view your medical history 24/7 through MyChart.

Log into MyChart

  • Medical records from a former Ascension location will be available in MyChart starting November 8, 2025.
  • If you need medical records from a former Ascension location prior to November 8, 2025, submit an Online Records Request or a paper Medical Records Release Form.

How to request copies of your records

Medical records can be released to you or someone you authorize. Processing takes 7–10 business days, and fees may apply.

Online medical records requests

To request records electronically online, you’ll need:

  • A valid email address
  • A cell phone
  • A driver’s license or government-issued photo ID

Required Information:

  • Patient’s full name, date of birth, address, phone (include previous names)
  • Recipient name and address
  • Specific records requested (location, provider name, type of report, dates of service)
  • Signature of patient or legal representative. Authorizations signed by a representative must contain a copy of the guardianship papers or power of attorney.
  • Date of signature

Requesting deceased patient medical records

To request a deceased patient’s medical records, complete both a medical records release form and the deceased patient medical records form. Both forms are available at the link below:

Send the completed and signed forms to the email, fax number or mailing address listed above.

How to get birth and death certificates

To obtain a copy of a birth or death certificate contact the county clerk’s office in the county the birth or death occurred or from the Michigan Department of Health and Human Services.
